Care transitions are where clients are most likely to fall through the cracks. A hospitalization or ER visit that the behavioral health provider does not learn about until much later, if at all, means a missed opportunity to follow up while it still matters. Real-time visibility into those events is one of the clearest ways to reduce gaps in care after a hospital stay or crisis encounter.
What this is
The Connex ADT (Admission, Discharge, Transfer) Dashboard gives providers access to ADT events from connected data sources, filtered to the clients the county actually serves, using criteria like patient roster or geographic attribution, so the alerts a provider sees are actually relevant to their caseload. When a matching event occurs, designated staff receive a real-time alert, so follow-up can happen while the transition is still fresh rather than being reconstructed after the fact.
Counties can also add ADT interfaces beyond the baseline connections included with Connex, extending visibility to additional third-party partners as needed.
